Only the school nurse shall administer medication to be taken during school hours. Medicines are administered to the student only with written permission from the doctor or the parents. Such medicine must be in the original container and delivered to the principal, school nurse or a designee from the Administration by the parent/guardian of the student. Medication must be accompanied by signed School administration Medication Permission Form. Medication Permission Form will be kept on file in the school clinic and must be renewed every school year. Any prescription drugs required by a student during school hours must be given to the nurse complete with the student’s name, class, time, and dose to be given.

Required Medical Examinations

All new comers must have a complete medical exam done by a physician. They must also complete a Medical History Form and update it yearly.
